INSTRUCTIONS

2. Circle the formation you want to choose. Available formations are 1-1-2, 2-1-1 and 1-2-1. Choose carefully and wisely.

3. Write the code of the players at the ‘Code Player’ column.

4. Then, write the positions, the value of the players including the total of your expenses.

5. It does not matter if you want to use all your £60 million or not as long you did not exceed the budget u received.

6. All forms can be sent to AL (10-04) or Reza (15-02).

7. Each participant can only sends a maximum of 3 application forms.
